Case Manager
Location: Alameda, CA
Work Hours: Monday to Friday, 9 AM to 5 PM
Pay Rate: $100K–$120K per year
We’re looking for a Case Manager to join our in-office team, supporting clients who need help organizing services and staying on track. This role involves building one-on-one relationships, staying organized, and making sure referrals and resources actually turn into results.
Day-to-Day Responsibilities:
- Work with clients to create manageable, realistic care plans
- Coordinate services, referrals, and appointments
- Maintain accurate and up-to-date files
- Follow up with clients and providers regularly
- Adjust plans as needs shift over time
Requirements
- Bachelor’s degree in social work, nursing, psychology, or similar field
- 2+ years of experience in case or care coordination
- Strong organizational and record-keeping abilities
- Working knowledge of local service systems
- LCSW, LMSW, or RN license is a plus
